JERSEY CITY, NJ (www.NJCUGothicKnights.com) | New Jersey City University's office of athletic training has prepared all required paperwork and guidelines to streamline the student-athlete physical process for the 2011-12 season. In order to play a sport during the 2011-12 season, all physicals must be conducted on one of the two dates listed. Physical Dates (at the Thomas M. Gerrity Athletic Complex team house):
- Monday, August 1, 2011 (8 a.m. to 10 a.m.)
- Monday, August 8, 2011 (1 p.m. to 3 p.m.).
All student-athletes must complete the 2011-12 information sheet and medical history form and are recommended to fill out the form, downloadable from this page, prior to their appointment.
No forms will be available on site; you must print the forms and bring with you.
